When am I eligible to enroll?

New Hires are eligible for benefits on their date of hire based on their role and Full-time/Part-time status in LCG360.

Enrollments MUST be done within 14 days of your hire date!

When does coverage begin?

Benefits will take effect retroactively to your date of hire.

Are you enrolling dependents?

If you are adding any qualified family members to any benefits you will need their DOB and Social Security Number in order to complete enrollment. You will also need to provide proof of their eligibility as follows.

Upload the following proof to the Document Record section in your LCG360 account.
• Spouse: marriage certificate or most recent year tax return
• Domestic Partner: proof of 6 months residing together, proof of financial dependency, and completed domestic partner affidavit
• Child: birth certificate, adoption papers, or custody court documents

After you enroll.

Once you have confirmed your enrollment, you can expect to receive insurance ID cards within 7-10 business days. Be sure to provide any documentation needed for your dependents, or your dependents will remain in pending status.

All benefit plans are governed by master policies, contracts, and plan documents. In the event of any inconsistency between the information contained herein and the applicable plan documents, the provisions of the plan documents shall prevail. Learning Care Group reserves the right to amend, suspend or terminate any benefit plan, in whole or in part, at any time. The authority to make such changes rests with the Plan Administrator.

Learning Care Group permits insurers to offer employees of Learning Care Group certain voluntary insurance programs. Whether you choose to enroll in any of these programs is completely optional and voluntary. Learning Care Group does not make a contribution towards the cost of these programs and employees pay the full cost of premiums on an after-tax basis. Learning Care Group does not sponsor, maintain, endorse, recommend, or promote these voluntary programs. Learning Care Group's involvement regarding these voluntary insurance programs is strictly limited to allowing the insurer access to employees to publicize these programs and Learning Care Group may perform certain ministerial functions such as payroll deduction and forwarding employee premium payments to the insurer. Learning Care Group does not receive any consideration in the form of cash or otherwise in connection with the program, other than reasonable compensation, excluding any profit, for administrative services actually rendered in connection with payroll deductions. Accordingly, these voluntary insurance programs are not subject to ERISA and related regulations. All questions or claims regarding these programs should be directed to the insurer.

A Summary of Benefits and Coverage (SBC) has been designed to assist you with better understanding the coverage being offered to you, and to allow you to compare coverage options.
