Qualifying Life Events

Qualifying Life Events (QLE) allow you to make changes within the year as long as you make the change and provide the required supporting documentation within the required timeframe. Please submit any QLE changes using the button below.

Make Changes

Qualifying Life Events (QLE) allow you to make changes to your benefit elections within the plan year if you make the changes and provide the required supporting documentation within the required timeframe.

If you experience one of the events below, you must report the event using the button below within 31 days of the date of the event. You will have an additional 30 days to provide the proof. If you miss either deadline, you may have to wait until the next Annual Open Enrollment period to make changes.

Spouses

Getting married, legally separated, or divorced is a QLE that qualifies you to make changes to your benefits.

Dependents

If you and/or your dependent are enrolled in a non-Gastamo plan and lose that coverage, you’re eligible for a QLE.

Children

Giving birth to or adopting a child or obtaining legal custody of a child qualifies as a QLE.

Add/Adjust/End

Things like Parking, Transit and Daycare benefits.

Gain of Other Coverage

Cancel and/or remove a spouse and/or dependents from coverage.

Change in Eligibility

Status change to FT or PT or from Regular to Temporary/Seasonal.

If the QLE and proof are received within the time frames the changes will be effective retro to the date of the QLE. If there are any missed premiums will be collected on a future paycheck. If there is a reimbursement due for premiums taken which were not owed a refund will be issued on a future check.

Note: If you are adding any qualified family members to any benefits you will need their DOB and Social Security Number in order to complete enrollment. You will also need to provide proof of their eligibility as follows.

Upload the following proof to the Document Record section in your LCG360 account.
Spouse: marriage certificate or most recent year tax return
Domestic Partner: proof of 6 months residing together, proof of financial dependency, and completed domestic partner affidavit
Child: birth certificate, adoption papers, or custody court documents

All benefit plans are governed by master policies, contracts, and plan documents. In the event of any inconsistency between the information contained herein and the applicable plan documents, the provisions of the plan documents shall prevail. Learning Care Group reserves the right to amend, suspend or terminate any benefit plan, in whole or in part, at any time. The authority to make such changes rests with the Plan Administrator.

Learning Care Group permits insurers to offer employees of Learning Care Group certain voluntary insurance programs. Whether you choose to enroll in any of these programs is completely optional and voluntary. Learning Care Group does not make a contribution towards the cost of these programs and employees pay the full cost of premiums on an after-tax basis. Learning Care Group does not sponsor, maintain, endorse, recommend, or promote these voluntary programs. Learning Care Group's involvement regarding these voluntary insurance programs is strictly limited to allowing the insurer access to employees to publicize these programs and Learning Care Group may perform certain ministerial functions such as payroll deduction and forwarding employee premium payments to the insurer. Learning Care Group does not receive any consideration in the form of cash or otherwise in connection with the program, other than reasonable compensation, excluding any profit, for administrative services actually rendered in connection with payroll deductions. Accordingly, these voluntary insurance programs are not subject to ERISA and related regulations. All questions or claims regarding these programs should be directed to the insurer.

A Summary of Benefits and Coverage (SBC) has been designed to assist you with better understanding the coverage being offered to you, and to allow you to compare coverage options.
